Overview

Northern NSW Local Health District has recently introduced a number of new services as part of the Leading Better Value Care (LBVC) Program.

These new services complement existing services designed to support ongoing management of people with chronic disease within our region, and include:

  • High Risk Foot Service
  • Knee and Hip Arthritis Service (KAHAS)
  • Renal Supportive Care (RSC)
  • Osteoporosis Refracture Prevention Program.

This event is designed to ensure that participants have a clear understanding of the 4 LBVC services in our region, referral pathways and evidence base behind these services.

Participants will have an opportunity to engage and ask questions of the local service providers.

Learning Outcomes

By attending this workshop, participants will have increased their confidence, knowledge and skills to enable them to:

  • Explain to patients and colleagues the services offered by:
    • High Risk Foot Service
    • Knee and Hip Arthritis Service (KAHAS)
    • Renal Supportive Care (RSC)
    • Osteoporosis Refracture Prevention Program.
  • Explain the evidence base underpinning these services.
  • Refer patients appropriately to these services based on service-specific indications for referral.
  • Use HealthPathways to assist with assessment, management and referral of patients eligible for these services.

High Risk Foot Complications: Prevention, Recognition & Management

Overview

In Australia and internationally, ‘high risk’ foot complications are most commonly associated with diabetes. These complications are a major source of suffering and expense for patients and a considerable financial burden on the health system. The average life expectancy of a diabetic patient following a foot amputation is 2 years.

General practitioners, primary health nurses and allied health professionals are invited to attend this free interactive workshop focused on the ‘high risk’ foot and strategies to prevent associated complications.

The format of the workshop will include a mix of:

  • Presentations including an overview of the High Risk Foot Service and local referral pathways available in the management of the ‘high risk’ foot.
  • Learning stations to develop skills in conducting a foot assessment and advances in medical devices and education in the management of diabetes.

Learning Outcomes

By attending this workshop, participants will have increased their confidence, knowledge and skills to enable them to:

    • Identify multidisciplinary interventions in the prevention of ‘high risk’ foot complications.
    • Perform a foot assessment to determine a patient’s risk of foot complications.
    • Describe local referral pathways for people with diabetes and people at risk of foot complications.
    • Describe services provided by the Clarence Valley High Risk Foot Service.
    • Develop relationships with local health professionals involved with the High Risk Foot Service.
